Once your payment is made, please return to this page to schedule your appointment, and finally, complete and submit the forms below.

Please select the “Schedule your appointment” button below and select an available date and time from the calendar provided.

Each session lasts 45 minutes, which means that a session that begins at 7am will end at 7:45am.

Enter your name and contact information as requested. You may also select the time zone if necessary.

Finally, select the SUBMIT button to complete the appointment. Dr. Nivischi will confirm within 24 hours.

Please submit both forms. The first is an informed consent. The second is a personal inventory questionnaire. These forms will help me understand how best to meet your needs.
